Patients may request medical records by submitting a written request to our office staff. While you may download records in Athena, they are typically more limited than what staff is able to manually compile. Once the request is received, our team will compile the records and provide instructions for release. A $20 administrative fee applies to records requests made directly to patients or third parties. However, if records are being sent directly to another healthcare provider for continuity of care, there is no charge. Records can often be transmitted electronically to the receiving provider.
